If your student has a food allergy, the food service director must have a signed medical statement by a recognized medical authority. 

The medical statement must include:

• an identification of the medical or other special dietary condition which restricts the child's diet;
• the food or foods to be omitted from the child's diet;
• the food or choice of foods to be substituted.

Please contact the food service director at 838-3662.  If you do not want a substitution made, but would like us to be aware of an allergy, please contact us.
